Different approaches to volume assessment of lymph nodes in CT Scans of HNSCC in comparison with a real gold standard

چکیده
Objectives: Volume assessment in head and neck squamous cell carcinoma (HNSCC) becomes an increasing important clinical parameter in treatment planning and response control. Various authors showed a significant impact of tumor volume on outcome and local control. In this study we compared the “gold standard”, the diameter based approach, with segmentation based approaches Methods: The segmentations were based on CT scans of 4 patients with HNSCC undergoing neck dissection as part of their treatment. These scans were taken during staging and a total of 20 lymph nodes were segmented. They were selected by size and location close to reproducible anatomical structures. The true volumes were measured by water displacement. Results: Pearson’s correlation index shows a higher correlation of the diameter generated volumes (r=0.723) than results generated via segmentation (r=0.527) with the true volumes. Nonetheless diameter generated volumes show clearly too high volumes at 146.8% (Confidence Interval: 115.8% – 186.1%). Volumes generated with the segmentation are at 116.5% (Confidence Interval: 93.9% – 144.7%). Conclusions: The data show a higher reliability of volumes estimated by the segmentation based approach than the widely used diameter based approach.

Diagnostic Accuracy of CT Scan for Detection of Cervical Lymph Node Metastasis in Oral Squamous Cell Carcinoma in Comparison with Histopathological Analysis After Neck Dissection
Objectives: Presence/absence of cervical lymph node metastasis plays a critical role in prognosis and survival of patients with oral squamous cell carcinoma (SCC). This study was designed to assess the diagnostic accuracy of computed tomography (CT) scan for detection of cervical lymph node metastasis in oral SCC in comparison with histopathological analysis after neck dissection Methods: In...
متن کامل99mTc-DMSA (V) in detection of metastases of medullary thyroid carcinoma
Introduction: Medullary thyroid carcinoma (MTC) is a rare thyroid cancer secreting calcitonin (CT) which is the most sensitive and specific tumor marker for MTC. This type of thyroid cancer is able to metastasize to different body areas including regional lymph nodes, lungs, liver and bone. The aim of this study was to assess the sensitivity and specificity of [99mTc-DMSA (V)] whole body ...
متن کاملA Comparative Study of CT Scan and MRI in The Assessment of Squamous Cell Carcinoma of The Oral Cavity and Its Cervical Lymph Node Metastasis
Introduction: Squamous Cell Carcinoma (SCC) is the most common head and neck malignancy. To decrease the side effects of treatment and mortality from disease relapse, careful staging for the proper treatment plan is necessary. The main purpose of this prospective study is to compare the diagnostic value of Computed Tomography (CT) and Magnetic Resonance Im-aging (MRI) in oral SCC and its lymph ...

Background Radiation dose about 400 times that of standard thoracic computed tomography (CT) in comparison with chest X- ray resulting in different approaches to decrease radiation dose have been established in the last few years to prevent possible side effects especially in children, such as low dose protocols. The aim of this study was assessment of clinical v...
متن کاملApproximation of head and neck cancer volumes in contrast enhanced CT
BACKGROUND Tumor volume may serve as a predictor of response to radiochemotherapy (RCT) in head and neck squamous cell carcinoma (HNSCC). Computer assisted tumor volumetry requires time-consuming slice-by-slice manual or semi-automated segmentation. We questioned how accurately primary tumor and suspect cervical lymph node (LN) volumes can be approximated by the maximum tumor diameters in three...
